Enrollment Flow

Admission and Enrolment Procedure for Undergraduate Students

 

A. For Incoming Undergraduate Freshmen, Transferees (MSU and Non-MSU Campuses, and Technology Students)

STEP 1: Screening of Requirements – Office of Admissions

  • Present the original and xerox copies of the following documents:
    • MSU-SASE/CET or CBP Rating Report
    • Senior High School Report Card/Form 138A
    • Certificate of Good Moral Character
    • PSA-SECPA Birth Certificate
    • Signed Evaluation Sheet or Transcript of Record (for Transferees and Second Degree Pursuers only)
    • Transfer Certificate (for Transferees only)
    • ID Photos

STEP 2: Pre-admission – Admitting College or Department

  • Go to your chosen College/Department for Pre-admission
  • Submit your credentials for pre-screening and registration
  • Secure signed Pre-admission Confirmation Slip (PCS) from the Academic Program Adviser

STEP 3: Admission – Office of Admissions

  • Proceed to the Office of Admissions for Approval of pre-screened credentials
  • Present your signed Pre-admission Confirmation Slip (PCS)
  • Secure your University ID number and take a photo from the designated admission officer assigned to your college

STEP 4: Pre-registration – Admitting College or Department

  • Return to your Academic Program Adviser
  • Accomplish your College Pre-registration Form (CBRF) or Pre-enrolment Form (PEF)
  • Submit your accomplished CBRF/PEF for encoding of courses in the database
  • Print your Enrolment Billing Form (EBF)

Once enrolment is confirmed, the student cannot add or delete courses in his/her database. NO REFRESH.

STEP 5: Payment of School Fees – University Business Office (UBO)

  • Present your signed EBF to the designated UBO personnel for payment of other school fees
  • Secure the Official Receipt (OR) given by the designated UBO personnel

STEP 6: Medical Examination – Medical Services and Hospital Division (MSHD)/ University Infirmary

  • Take a medical examination at the University Infirmary
  • Secure your Medical Certificate from the designated University Infirmary personnel

STEP 7: Confirmation of Enrolment – University Registrar

  • Present your EBF and OR to the designated University Registrar personnel
  • Secure your Certificate of Registration (COR) for final validation and approval of enrolment
  • Check your enrolled courses and its section on your AKAN account

END OF TRANSACTION

 


B. For Old Undergraduate Students (Shifters and Non-shifters)

STEP 1: Academic Advisement – Division of Student Affairs (DSA)

  • Undertake academic advisement from the designated DSA personnel
  • Accomplish and secure your Shifting Form from the designated DSA personnel
  • Print your Evaluation Sheet from the University Registrar

*Step 1 is applicable only to shifters. Non-shifters should proceed to Step 3.

STEP 2: Department Screening – Admitting College or Department

  • Take the Department Entrance Examination (written and/or oral) in the chosen college/department
  • If you pass the college/department screening, proceed to Step 3

STEP 3: Pre-registration – Admitting College or Department

  • Present your signed Evaluation Sheet and accomplished Shifting Form to the Academic Program Adviser
  • Accomplish your College Pre-registration Form (CBRF) or Pre-enrolment Form (PEF)
  • Submit your accomplished CBRF/PEF for encoding of courses in the database
  • Print your Enrolment Billing Form (EBF)

Once enrolment is confirmed, the student cannot add or delete courses in his/her database. NO REFRESH.

  • Present your signed EBF to the designated UBO personnel for payment of other school fees

C. For College Bound Program (CBP) Students

STEP 1: Screening of Requirements – Office of Admissions

  • Present the original and xerox copies of the following documents:
    • MSU-SASE/CET Rating Report
    • Senior High School Report Card/ Form 138A
    • Certificate of Good Moral Character
    • PSA-SECPA Birth Certificate
    • 2×2 recent ID Photos with mame-tag
    • 1 Long Brown Envelope (labelled with name in bold letters)
  • Accomplish the CBP Admission Form 1
  • Submit the accomplished CBP Admission Form 1 to the Admitting Officer for the issuance of ID number and database encoding

STEP 2: Payment of Training Fee – University Business Office (UBO)

  • Pay the Training Fee to the designated UBO personnel
  • Secure the Official Receipt (OR) given by the designated UBO personne

STEP 3: Confirmation of Enrolment – Office of the Admissions

  • Present your OR to the Enrolling Officer for enrolment and confirmation of CBP subjects (English, Math, and Science)
  • Proceed to Window 6 for printing and releasing of Certificate of Registration (COR)

END OF TRANSACTION

 


D. Admission procedures for both new and old students of College of Law, College of Medicine, and Graduate School are prescribed by the foregoing colleges.
